Health Insurance Refund

Form Download
  1. Complete student request form, with current address, ID#, phone number, and email address.
  2. Please indicate the term your requesting a refund, and the reason for the refund. The two justifications below are the only two that will be accepted for a refund.
    • Your leaving the country permanently. (You must provide copy of your airline ticket showing the date of your departure.)
    • The company you are working for is providing insurance. (You must show proof of insurance coverage.)
    • Leaving CSUSB to study at another university. (Must speak to Elanor Perry and fill out a F-1 Transfer Form)
  3. Please make clear the address you want your refund mailed to.
  4. Once your request is submitted, it will take approximately 6-8 weeks to receive refund.
